What are dermal fillers?

Dermal fillers are a group of closely related injectable, cosmetic products used predominantly in facial aesthetics to fill lines and wrinkles or to give lift and volume, define, contour and sculpt mid and lower facial areas. Dermal fillers can also help rejuvenate skin through deep hydration and can visually improve scarring too. A dermal filler is classified by UK MHRA as a medical device, i.e. a product that is used for medical purposes. However, dermal fillers differ from medicines such as BOTOX® in that they do not perform any medical function on the body.

 

Hyaluronic acid dermal fillers?

The active ingredient in the most commonly used dermal fillers is hyaluronic acid (HA) which occurs naturally in the body. Approximately 56% is found in the skin. Hyaluronic acid is essential in holding together collagen and elastin fibers within the dermis, giving the skin its natural, youthful looking elasticity to accommodate movement of joints and muscles. As we age the molecular composition of the skin changes as does the density of hyaluronic acid contained within the skin. The resulting loss of volume exhibits the characteristic lines and wrinkles, sagging cheeks, jowls and jaw line associated with ageing.

 

How long do dermal fillers last in the body?

Most dermal fillers are manufactured from synthetic hyaluronic acid. These dermal fillers provide temporary improvement to the treated areas. The longevity of each treatment will depend on several factors: i) the area treated ii) the health and properties of the skin tissues being injected iii) the type and quantity of product iv) the lifestyle of the person (smoking, alcohol, drugs, UVA sun exposure, sun beds, sugars in diet, nutritional deficiencies, etc) v) certain biological factors such as age, well being and metabolism. As a guide, it is reasonable to expect up to 6 months longevity from a single treatment where sufficient product has been correctly injected and where the patient is a normal healthy adult. For treatment areas where there is higher vascular activity such as the lips and for areas which are more expressive e.g. around the eyes then longevity may be reduced.

 

Can dermal filler be removed?

Hyaluronic acid based dermal fillers such as Júvederm, Restylane, Teosyal, Belotero, Intraline or skin boosters such as Profhilo can be dissolved using a product called hyaluronidase. Hyaluronidase can reverse the process of dermal filler integration by breaking it down so that it may be safely eliminated from the body. Hyaluronidase is a prescription only medicine and should only be supplied by a registered medical professional. It must only be administered by an experienced medical aesthetics practitioner. The product must be delivered in a controlled manner as it has the potential to dissolve naturally occurring hyaluronic acid in the skin.

 

Other dermal fillers which are not hyaluronic acid based cannot be dissolved. These products include fillers manufactured from calcium hydroxylapatite such as Radiesse or from Poly L-Lactic acid such as Sculptra. These products will break down over a longer period of time (possibly up to 3 years). It is important to consider the properties of the injectable product before undergoing any proposed treatment.

 

Advanced dermal filler treatments – Who can you trust?

Our clinic is widely respected throughout the UK for our specialist skill in advanced and complex dermal filler procedures. Sadly, over the past 2-3 years we have seen a sizable increase in clients from other clinics requiring corrective dermal filler procedures. It is imperative to judge a practitioner’s specialist knowledge and ability to perform each individual dermal filler procedure. A poorly performed dermal filler treatment could result in misplaced product or complications such as a temporarily deformed face, facial asymmetry and even prolonged pain or discomfort.

 

An experienced doctor, dentist or nurse cannot be relied upon to perform advanced dermal filler treatments if they have not had specific medical aesthetics training. It is essential to check your practitioner’s medical aesthetics training and qualifications for the exact procedure being undertaken. If necessary ask to see their certificates and some real examples of their work before consenting to the proposed procedure.

Types of dermal filler

Juvéderm

Juvederm An extensive range of FDA approved injectable, gel based facial fillers. Licensed to Allergan Ltd, UK, a division of Allergan inc. California, USA. Thanks to its advanced cross linked formula Juvederm has been proven to last for over 9 months. The Juvederm range includes Juvederm Ultra for lines and wrinkles, Juvederm Ultra smile specially formulated for lips, Juvederm Voluma for advanced volume enhancement and contouring of cheeks, jaw and the non surgical facelift. There is also Juvederm Hydrate for advanced skin rejuvenation and the recently acclaimed Juvederm Volbella and Volift both of which are proven to last beyond 12 months and possibly up to 18 months.

 
Restylane

Restylane is one of the most widely recognised facial aesthetics products in the world and the first to gain FDA approval. The Restylane brand also includes Perlane and Emervel. Over 16 million Restylane treatments worldwide have been performed to date. It is a long lasting cross linked dermal filler. Restylane is a trademark of Q-Med AB, a Galderma division.

 
Belotero

Belotero is an innovative cross linked dermal filler with excellent distribution properties thanks to its unique manufacturing process. Licensed to Merz pharma UK, a division of Merz Pharma GmbH & Co, Germany. Belotero dermal fillers also have FDA approval.

 

Belotero is available as Soft, Balance and Intense covering all lines and wrinkles up to advanced facial contouring, lifting and sculpting.

 
Teosyal

Teosyal is a long lasting cross linked, gel based hyaluronic acid dermal filler with CE approval and FDA approval pending. It is manufactured by Teoxane Laboratories, Switzerland and distributed by Lifestyle Aesthetics in the UK. Teosyal is a stabilised, non animal based hyaluronic acid with excellent hydration properties. The filler is 100% non allergenic. Clinical tests show Teosyal may last for up to 12 months. A range of 8 different fillers are available in addition to the highly acclaimed Redensity II – the world’s only approved tear trough filler.

 
Dermal fillers versus BOTOX®

Whilst some similarities exist between BOTOX® and dermal fillers their physical properties are very different, as are the functions each performs in relation to skin. Botulinum toxins such as BOTOX®, Bocouture and Azzalure temporarily relax the muscles whose contractions are responsible for lines and wrinkles in the skin. Dermal fillers have no effect on facial muscles, instead they replace lost volume in the dermis and may also stimulate collagen.

 

It is not uncommon to use both BOTOX® and dermal fillers together to address the same area of wrinkles, particularly where both muscle contractions and loss of collagen, elastin, fat, or other tissue are responsible.

Are dermal fillers safe?

There are about 190 cosmetic dermal fillers used in the UK and across Europe. Only 14 are FDA licensed for cosmetic use. Licensed dermal fillers have undergone extensive scientific testing to ensure their safety, tolerance and effectiveness.

 

Side effects with dermal fillers are rare and particularly when handled by a suitably knowledgeable and qualified person. Where they do occur they are generally mild and diminish within a few days. The most common side effects are swelling, redness, bruising and mild pain or discomfort at the injection site.
